First a disclosure - In the past I formerly taught math at the college level and am not presently in the classroom.

At first blush it seemed like it might be a good idea, to get some unbiased feedback, something not unlike an anonymous survey.

But upon closer inspection, most students do not provide constructive criticism that a teacher could use, and even the options for rating a teacher distill it all down to a popularity meter or rating of sorts.

I think a better approach would be to have an evaluation form passed out in class that became part of the record, and allowing for further questions and discourse surrounding recommendations as opposed to anonymous postings on a web site that may or may not be sincere in their offering.
